      Please complete the following declarations. If this declaration is not completed, your application will not be complete and the named child WILL NOT be able to take part in any sessions.

    • Medication

      Medication that may be needed must always be brought along to all sessions and training e.g. inhalers. Failure to do so may result in the named child not being able to take part due to safeguarding and first aid protocol. Coaches will check every time.

    • Injury

      In the event of the named child being injured whilst taking part in sports sessions / training and I cannot be contacted on the above numbers, I hereby give my consent for the named child to receive emergency medical attention. I support this application and understand that Boston United FC Community Foundation and its coaches accept no liability in respect of injury, loss or damage to property or persons.

    • First Aid

      Please tick here to confirm you give Boston United FC Community Foundation and its coaches consent to administer first aid to the named child, and if necessary, transfer them to hospital should an emergency arise whilst within the care of Boston United FC Community Foundation.

    • Protective Gear

      Please tick here to confirm that you understand that for your child to take part in our sessions, they must bring and wear the appropriate protective gear. For example, shin pads, goalkeeper gloves and so on. If they do not wear the appropriate protective gear, they will not be allowed to take part.

    • Photography and Video Consent

      Boston United FC Community Foundation may take photographs and/or videos at events and activities. These will be used for promotional and marketing purposes; such as promotional flyers, brochures, social media, on our website and event / fundraising resources. We will never publish a photo or video with personal contact details (e.g. address, contact number, email). By giving us photography and video consent, you will help us to promote Boston United Football Club and Boston United FC Community Foundation positively.
